Vitamin B12 Deficiency Megaloblastic Anemia After Tota1 Gastrectomy (2 cases)

Abstract
Megaloblastic anemia due to cobalamin deficiency can occur as a late sequale of total
gastrectomy because of inadequate or absent secretion of intrinsic factor. We presented
two cases of vitamin B12 deficiency megaloblastic anemia after
total
gastrectomy in 59-year -old male and 55-year -old female, respectively. They were
diagnosed as stomach cancer and total gastrectomy with esophagojejunostomy were
performed 8 years ago.
They were relative well until 4 months, when they had complaints of headache and
dizziness. They revealed anemic symptoms and signs, but no neurologic
problems.
Laboratory findings showed macrocytic anemia, thrombocytopenia, markedly decreased
serum vitamin B12 levels and increased LDH. The findings of peripheral
blood and bone marrow were compatible with megaloblastic anemia. After treatment of
parenteral vitamin B12, their clinical and hematologic features improved
progressively and they have been followed up on OPD basis with maintenance therapy
of vitamin B12.
